Social Services Assessment Form

Our Social Services Coordinator, Ana, is available for patrons to meet with for 20-minute consultations to help refer patrons to services. To meet with Ana, submit an assessment form.        

(Appointments can be virtual, in-person or, over the phone. Alternatively, resources can be sent through email after filling out thisassessment form.)

What Ana does:

  • One-on-one advising and guidance.
  • Helping families and individuals by connecting them to useful and necessary state and local resources such as SNAP, Medicaid/Medicare, LIHEAP, IDHS, Social Security, and mental health or substance abuse counseling.
  • Assessment - evaluation, research, and therapeutic intervention with individuals, groups, and families.
  • Networking - assisting with community organization, program development, and services that address community needs.
  • Last but not least, she is here to listen and help in any way that she can. Come say hello!

Appointments at the Steger-South Chicago Heights Public Library provide brief visits for informational and referral services. Appointments are limited to 20 minute consultations and are free of charge
